ACEP keeps a record of your chapter's leaders. This information is due upon changes to chapter leadership including election of new Board members and/or Officers. Complete this form, save and submit here.
Chapter Membership Dues Change Form
Chapter membership dues are set by each chapter and billed to members by ACEP as part of the annual dues statement. Member dues changes must be authorized by the appropriate parties in compliance with your Chapter Bylaws and the decision must be recorded in the meeting minutes. Complete this form, save and submit here to request that ACEP update the amount of dues to be charged for specific categories of membership in the chapter.
Chapter PAC Collection Form 2024
If ACEP collectes chapter PAC contributions on behalf of your chapter, you are required to annually complete this form, save and submit here. ACEP will update the suggested contribution amounts for the specified categories of membership in the chapter.

Complete this form by September 1, 2023 for the 2024 calendar year.
Annual Meeting Exhibit/Product Theater Request Form
ACEP's Development Team is updating the 2023 Annual Chapter Meeting roster. This list helps us help you through soliciting support for Exhibit and/or Expert Theater (Product Showcase) opportunities for your Chapter. If you would like your chapter to be included on this list, please complete this application by Tuesday, October 24, 2023.
Quarterly Chapter Updates
Every quarter ACEP compiles and shares a report of recent chapter activities, accomplishments and challenges of note with the ACEP Board and staff. Chapter Executives are asked to provide a brief high-level paragraph or a bulleted list (10 items max) with links to your chapter website for more information (if applicable). Chapter updates are due on the following dates via this form.
